Job Description
Lifelong Access’s mission is to connect families and people with developmental disabilities to their community through a lifetime of meaningful supports. Lifelong Access provides a continuum of services and supports for families, children and adults throughout their lifetime including residential, vocational development, employment, high school transition and therapeutic supports for children. Lifelong Access envisions a community that encourages and accepts all people.
Lifelong Access is seeking a Case Manager to provide service facilitation for assigned individuals in Medicaid waiver programs (CILA, Community Day Services and Supported Employment). This is a crucial role in supporting individuals with intellectual and developmental disabilities to navigate various aspects of their lives. The primary focus is to enhance the quality of life for individuals with IDD by coordinating and advocating for the necessary services and resources.do!
Pay: Hourly; Based on experience
Job Type: Full-time
Primary responsibilities include:
  • Assessment and Planning, conduct initial and ongoing assessments to identify and understand unique needs, strengths, and challenges of each individual. (I.E. ICAP, SLOF, RISK, MEDICAL, SNAPSHOT, Etc..)
  • Collaborate with individuals, their families, and other professionals to develop holistic implementation strategies. 
  • Advocate for the rights and interests of individuals, ensuring that they have access to appropriate services, education, and community resources. (Consents) 
  • Work with other professionals and agencies to address any legal or rights-related issues affecting individuals. 
  • Coordinate and monitor the delivery of services to individuals, ensuring that their needs are met. 
  • Maintain accurate and up-to-date records of each individual’s progress and services received. 
  • Be prepared to handle and respond to crises or emergencies, providing immediate support and coordination of necessary resources. 
  • Work closely with other professionals, including social workers, healthcare providers, educators, and community organizations, to provide a comprehensive support system for individuals. 
  • Provide assistance and training in daily living skills to help individuals achieve greater independence. 
  • Develop and implement programs that focus on enhancing the individuals’ abilities to live in their communities. 
  • Facilitate community involvement and integration for individuals, promoting socialization and inclusion in various community activities. (Natural Supports)
  • Maintain open communication with individuals, their families, and support staff to address concerns, provide updates, and ensure a collaborative approach to care. 
  • Train direct care staff, providing guidance and support in implementing individualized service plans. 
  • Ensure compliance with relevant regulations, policies, and procedures governing the care and support of individuals with developmental disabilities. 
  • Provide training to staff, individuals, and families on relevant topics, including disability awareness, communication strategies, and crisis management. 
  • Complete and maintain accurate documentation, including progress notes, incident reports, and other required paperwork. 
  • Prepare and submit reports as required by regulatory agencies. 
  • Provide coverage in Medicaid waiver programs as needed.
Qualifications for this position include:
  • Bachelor's degree in psychology, sociology, education, counseling, or a related field.
  • Minimum of one years’ experience supporting individuals with disabilities required. 
  • Demonstrated ability to work within a community partner’s structure; and ability to work constructively with individuals receiving services, other providers of service, and the community.
  • Certified as Qualified Intellectual Disabilities Professional (QIDP), or eligible and able to complete certifications.
  • Proficient with Microsoft Office applications (Word, Excel, and Outlook).
  • Engaging hands-on leadership with effective written and verbal communication skills.
  • Excellent organizational skills and attention to detail.
  • Critical thinker, detailed, problem solver with high ethical standards.
  • Availability based on the needs of people supported, which could include days, evenings and/or weekends if necessary, depending on staffing.
  • Valid Illinois driver’s license with safe driving record. 
  • Current proof of automobile insurance.

While performing the duties of this job the employee is regularly required to talk or hear. The employee will also frequently need to converse with adults – some who might be difficult to understand or use nonverbal forms of communications. The employee is frequently required to sit, stand, and walk for periods of time. Also frequently required to use hands< fingers to reach and feel. Occasionally required to stoop, kneel, crouch, or crawl. Specific vision abilities required by this job include clear close and distance vision. Must be able to occasionally lift and/or move up to 20 pounds at times.
